I suspect not taking the top mount off my 5D MK III caused the hot shoe to become loose over time. I used the instructions here http://www.conraderb.com/flashrepair/ To access the screws and tighten it. It requires a bit of force as they use some glue to keep down the plate but it was easy to fix and I never glued it back.
